VERIFICATION FORM

Please complete a "Verification of Tutor Program(s)" for each program you want to be certified. For example, if you wish to have Program A certified at Levels 1 & 2, then you would complete the verification for Levels 1 & 2 for Program A's application. If you also have a Program B that you wish to have certified at Level 1, then you would complete a separate application for Program B, Level 1 that would also include a separate verification form.

Please refer to the document titled "CRLA'S REQUIREMENTS FOR CERTIFICATION OF TUTOR PROGRAMS" for a complete listing of all the
LEVEL 1, 2, AND 3 requirements for each program to be certified.

The "necessary documentation" called for under each criteria consists of patterns of evidence of the what, how, and when of the training program. It could/should include any or all of the following that can help the CRLA Tutor Certification Committee verify your program(s):

1) course syllabi at PCC   (referenced on front page of www.angelfire.com/blues/writing/CRLA.hmtl)

2) titles of textbook "The Master Tutor – A Guidebook for More Effective Tutoring, by Ross B. MacDonald, Ph.D."

3) sample flyers (Conversation Group tutoring) /posters/memos Writing Resources (Lab Tool)

4) sample worksheets: Communication: Ten Steps 

5) handouts, worksheets:  Communications:  Listening

The better documentation you provide the easier it will be for the committee to certify your program(s); however, brevity is appreciated due to mailing requirements, so condensed but complete documentation is requested.

Tutors are evaluated in accordance with the following procedures. The appropriate center personnel
re responsible for supervising and evaluating each tutor.  The formal evaluation process includes:
 

               1.  Tutor Evaluation and Self Assessment Tool (TESAT) after four and nine weeks of tutoring. Tutors will use the Tutor
                   Evaluation and self-Assessment Tool (TESAT). Tutors are expected to draft notes reflecting upon their progress
                   as a tutor and be prepared to discuss this and the results of the Tutee Feedback form during their supervision sessions.
               2.   Tutee Feedback Form ( "How Are We Doing At The Tutor Center" ) after four and nine weeks of tutoring.
               3.      For any tutor who conducts small group Tutoring Sessions, appropriate faculty members will observe one session each
                    semester and meet with the tutor immediately after the session.   
              
4.  Tutor Evaluation Report. The center coordinator will meet with each Tutor to discuss their self-evaluation and Tutee
                    Feedback Forms. Tutors receive a written evaluation reflecting the information discussed in the session. The results
                    of the evaluation are used to assist in tutor development and to focus further training/supervision topics to build
                    confidence and skills.
               5.   Tutors are asked to provide an evaluation of their experience as a tutor each year.  Student feedback is valued and
                     is essential to the decision-making process for determining needed program adjustments to enhance the quality
                     of our service.
